What sucked

At a large US health system, the first person an emergency department patient met was usually front-desk staff with no clinical training, and in some cases they helped decide who was seen first.

The job was hard and people did not stay, so training that ran for weeks kept restarting with each new hire. Triage was going wrong in both directions: some patients who needed to be seen quickly waited, while others were sent to high-priority pods they didn't need, taking beds from people who did. More training and closer monitoring had not fixed it.
